DO YOU TAKE INSURANCE?
We do not take insurance for your office visit, but you are welcome to file a claim on your own for possible partial reimbursement depending on your policy. However, all blood labs and referrals for imaging can still be filed to insurance. Test kits (hormones, cortisol, GI, neurotransmitters, heavy metals, mold, etc) are all cash pay and price varies by kit. Please call the office for exact pricing on kits.
WHAT SUPPLEMENT BRANDS DO YOU CARRY?
Numedica, Metagenics, Orthomolecular, Biogenetix, Bezwecken, Neuroscience, Xymogen, Quicksilver Scientific, MD Bio, LMNT, Clean Simple Eats
DO YOU HAVE PAYMENT PLANS AVAILABLE?
Yes, we offer our payment plans through Care Credit. You may also use Zip Pay to split your total into 4 or 8 payments.
HOW DO YOU TEST HORMONES AND CORTISOL?
We do salivary testing for male/female hormones and cortisol, as this method is more accurate than blood testing. Thyroid labs are still drawn via blood.
DO YOU OFFER TELEHEALTH?
Yes, we offer telehealth services to anyone interested. Whether you live out of state or locally, we can order labs anywhere in the United States as well as ship testing kits and supplements to your door.
WHAT AGES DO YOU SEE IN THE OFFICE?
The functional medicine side sees birth and beyond patients. Aesthetics can treat patients 15 years of age and older (parent consent needed for patients under 18).
DO YOU SHIP SUPPLEMENTS?
Yes, we can ship to anywhere in the United States. Shipping cost will depend on the size of the box needed to fit your order. We use USPS flat rate prices for shipping. Please call the office and they can give a shipping quote depending on the products you need sent.
DO YOU OFFER IMAGING SERVICES?
We can send over referrals for imaging anywhere if you are using insurance for imaging. For cash patients needing an ultrasound, please use the link https://www.mkdiagnosticultrasounds.com to book with Mercades Kolosha. She will be at Metamorphosis Wellness & Aesthetics twice a month to perform ultrasounds. If you are needing imaging done sooner than her first available appointment at our office, you are welcome to book at one of her other locations and we will receive the results.
